    Purchase your RX lenses and/or eyewear
    Complete your purchase at full retail price (and get reimbursed later).

    Download (and fill out) a claim from your carrier’s website
    Be sure to follow your vision plan’s instructions for requesting out-of-network reimbursement.

    Submit the form to your carrier with your itemized receipt
    Get your itemized receipt (from your account or by contacting us), make sure to attach it to your claim.
