Please submit this form to register your product(s) within 30 days of purchase for an extended warranty. Submitting this product registration extends your non-prescription* lens warranty to 1 year (normally 90 days) and introduces a protection plan for replacement glasses within a year of purchase if you break them!

*Prescription lenses are not covered by this warranty. Please let us know within 30 days of receipt if there is an issue with your prescription lenses so we can have them remade.
